Cost of Annual Membership
● Single Coverage $360/yr.
● Husband/Wife $680/yr.
● Parent/Child $620/yr.

● Each additional child $260/yr (Includes children under 18 years of age or full-time college students).
● Periodontal Maintenance Program $834/y.
Periodontal Maintenance Program
Includes everything on the established patient plan, but will have up to 4 periodontal maintenance cleanings instead of the 2 preventative care cleanings.This is a 20% or more Savings off Preventative Care!Plus receive an additional 15% off any one restorative procedure during plan year!
Established Patient Program
● 2 Annual Evaluations.● 1 Emergency Evaluation.● 2 Preventative Care cleanings (non-periodontal).● 1 Full Mouth Series of x-rays when due (taken every 3 yrs).● 1 set of Bitewing x-rays when due (taken yearly).● Fluoride varnish treatment.
Guidelines
● No yearly maximums or deductibles.● All membership fees are due in full at the time of sign up.● You are immediately eligible after signing up and payment is received.● All additional charges are due the day of service.
